Healthy Hearts is a 12-week healthy lifestyle programme designed to support preventative care for those at risk of heart-related conditions, as well as people living with or recovering from heart conditions.

Across the 12 weeks, participants take part in gentle, supervised physical activity sessions alongside educational workshops focused on heart health, lifestyle changes, and emotional wellbeing. The programme aims to support individuals on their journey to better long-term health in a safe, supportive, and social environment.

  • Day: Tuesday 

  • Time: 6-8pm 

  • Location: Nelson Health Centre, 220 Kingston Rd, London SW20 8DA 

  • Programme Structure: 45 minutes of supported physical activity & 45 minutes of educational workshop | Sleep, Nutrition, Mental Health, Hydration, Goal Setting, Alcohol & Smoking etc.

  • Eligibility Criteria: Individuals at increased risk of developing a heart condition. This may include those who smoke, have low levels of physical activity, or consume alcohol excessively, as well as individuals with existing health conditions such as high blood pressure, high cholesterol, type 2 diabetes, or obesity. It also includes those with a family history of heart disease, or anyone who has previously experienced a heart-related condition or cardiac event.
