Healthy Hearts

Healthy Hearts is a 12-week healthy lifestyle programme designed to support preventative care for those at risk of heart-related conditions, as well as people living with or recovering from heart conditions.

Delivered by AFC Wimbledon Foundation, the programme provides community-based support to help participants become more active, improve their eating habits, and build confidence in managing their health.

Across the 12 weeks, participants take part in gentle, supervised physical activity sessions alongside educational workshops focused on heart health, lifestyle changes, and emotional wellbeing. The programme aims to support individuals on their journey to better long-term health in a safe, supportive, and social environment.

  • Day: Tuesday 

  • Time: 6-8pm 

  • Location: Nelson Health Centre, 220 Kingston Rd, London SW20 8DA 

  • Programme Structure: 45 minutes of supported physical activity & 45 minutes of educational workshop | Sleep, Nutrition, Mental Health, Hydration, Goal Setting, Alcohol & Smoking etc.

  • Eligibility Criteria: Individuals at increased risk of developing a heart condition. This may include those who smoke, have low levels of physical activity, or consume alcohol excessively, as well as individuals with existing health conditions such as high blood pressure, high cholesterol, type 2 diabetes, or obesity. It also includes those with a family history of heart disease, or anyone who has previously experienced a heart-related condition or cardiac event.

Week 5 | Eat & Drink Well: Adapting your diet | Circuit Based Activity Session

Week 6 | Life Skills: Creating action plans | Physical fitness testing activities

Week 7 | Eat Well & Drink Well: Food marketing tricks and myths & thinking about drinking | Physical activity session with a sport theme

Week 8 | Sleep Well: Developing healthy sleeping habits | Relaxation Activity Session

Week 9 | Life skills: Noticing progress | Games Based Activity Session

Week 10 | Life skills: Managing setbacks | Strength and balance session

Week 11 | Wellbeing: Looking after your wellbeing | Physical Activity Session
